Allegheny Valley School District 2023-24 School Sports Physicals 

Dear Parents/Guardians:  

Sports physicals for the 2023-2024 school year will be, once again, offered by the Allegheny Valley School District. Dr. David Shin, the school physician, will be available for the sports physical exams.  Any student, entering grades 7-12, who is interested in playing a school sport during the 2023-2024 school year, is encouraged to schedule a physical. Allegheny Valley School District offers sports physicals free of charge. The physicals will be accepted for any sport, any season, for the 2023-2024 school year.  

DATE:       Saturday, June 10th, 2023.  

TIME:         8:00 am- 11:00 am

LOCATION:   ALLE KISKI MEDICAL ASSOCIATES Office,1304 Pittsburgh St, Cheswick

Please follow the TWO steps required to register your child for school sports physical:

1. Schedule your child for an appointment on the Google calendar.

DIRECTIONS to time slot in Google calendar:  

When the Google calendar appears, find the June 10th calendar date.  At this point, there will be appointment slots.  Please pick an appointment time and type your child’s name in the provided field. 

2. Register your child for family ID.  Family ID information is required BEFORE the physical date and time.

***Please note:  You may also choose to take your child to his/her own physician for the sports physical. However, Family ID information is still required.  All physicals for the 2023-24 sports season must be dated June 1, 2023, or after, to allow your child to participate in the sports season for next school year.
